We care about you and your family’s health!

In addition to the health care services provided at doctor visits, Community Health Group offers health education classes to help our members learn about health topics.

Some examples of health education classes are nutrition, diabetes, high blood pressure, family planning, and asthma. There are many more available as well. These classes are at no cost to our members.

Diabetes Prevention Program (Skinny Gene Project)

Community Health Group is working with the Skinny Gene Project to offer our members a diabetes prevention program, at no cost! Members can call the Skinny Gene Project to see if they are eligible.

  To be eligible, Cal MediConnect members MUST:

  •  Be enrolled in Medicare Part B
  • Have a body mass index (BMI) of at least 25, or at least 23 if self-identified as Asian
  • Meet one of the following three blood test requirements within the last 12 months: 
  1. A HbA1c test with between 5.7 and 6.4%, or
  2. A fasting plasma glucose of 110-125 mg/dL, or
  3. 2-hour plasma glucose of 140-199 mg/dL (oral glucose tolerance test)
  • Not be diagnosed with type 1 or type 2 diabetes (other than gestational diabetes)
  • Not have end-stage renal disease 

To be eligible, Medi-Cal members MUST:

  • Be a current Medi-Cal member with Community Health Group
  • Be 18 years old or older
  • Not pregnant
  • Have a body mass index (BMI) of at least 25, or at least 23 if self-identified as Asian
  • Meet one of the following three requirements:
  1. Blood test results within the 12 months of the first core session
  • A hemoglobin A1c test with a value between 5.7% and 6.4%, or 
  • A fasting plasma glucose of 100-125 mg/dL, or
  • A 2-hour plasma glucose of 140-199 mg/dL (oral glucose tolerance test)

  2.  Clinically diagnosed gestational diabetes mellitus during a previous pregnancy

  • Not be diagnosed with type 1 or type 2 diabetes (other than gestational diabetes)

Please note: These screening tools are not designed to provide a comprehensive assessment or diagnosis. Only a qualified physician or mental health provider can provide a complete assessment and diagnosis.
