Community Health Group Announces Upcoming Long-Term Care Provider Office Hours
Community Health Group (CHG) is committed to supporting our provider network as we implement the Department of Health Care Services' updated Long-Term Care (LTC) requirements under:
APL 26-010 – Skilled Nursing Facilities (SNF)
APL 26-011 – Subacute Care Facilities
APL 26-012 – Intermediate Care Facilities for Individuals with Developmental Disabilities (ICF/DD)
