What's Covered

Emergency medical transportation

Emergency transportation (ambulance) or ambulance transport services, provided through the “911” emergency response system, will be covered when medically necessary. No authorization is required.

Non-Medical Transportation Non-Medical

Transportation (NMT) is covered for medically necessary covered services. NMT is transportation by car, taxi, or other public or private way of getting to your medical appointment. Please call Community Health Group at 1-800-224-7766 for assistance.

Non-Emergency Medical Transportation

Non-Emergency Medical Transportation (NEMT) is covered for medically necessary covered services. NEMT is transportation by ambulance, litter van, wheelchair van, or air. Your primary care physician (PCP) or specialist will need to complete a Provider Certification Statement (PCS) form prior to receiving transportation. 

Arriving on Time to Scheduled Medical Appointment

NEMT and NMT trips will be scheduled so that you arrive within 15-minutes of the scheduled appointment time. CHG will arrange alternative transportation if the provider does not arrive at the scheduled pick up time.

Please contact CHG’s Member Services Department by calling 800-224-7766 if your driver for your authorized transportation is late or does not show-up. A Member Services representative will assist you in coordinating an alternative transportation service, which may include:

  • A different driver
  • Mileage reimbursement if your family member, neighbor, or friend can give you a ride

Our Member Services Department will work with your provider to let them know you may be running late for the appointment. 

Vision Benefits

Vision benefits are managed directly with VSP (Vision Services Plan). To find out more information about what’s covered, call us at 1-800-224-7766 .  You can also call VSP at 800-877-7195, they are available M-F 5AM to 8PM PST and S-S 7AM to 8PM PST.

Dental Benefits

Dental benefits are managed directly with the Medi-Cal Dental Program . To find out more information about what’s covered, call us at 1-800-224-7766 .  You can also call Denti-Cal at 800-322-6384. They are available M-F 8AM to 5PM PST.
