Who is Community Health Group (CHG)?

Community Health Group has been providing quality health benefits for San Diego County residents since 1982. As San Diego's largest locally based, non-profit HMO, we invite you to simplify your health care needs with Community y Más. Frequently Asked Questions - Community y Más

Am I eligible for Community y Más? 

If you live in San Diego County and you are receiving Medicare benefits (have both Part A and Part B), and you have one of these chronic conditions, Cardiovascular Disorders, Chronic Heart Failure, and/or Diabetes, you can sign up for Community y Más.

Can I still go to my regular doctors?

Yes, as long as your doctors are part of our Community y Más' provider network. Community y Más members will have access to many hospitals in San Diego County. 

Does Community y Más include prescription drug benefits? 

Yes. As a member of Community y Más, you will have the same drug benefits you receive under Medicare Part B and under the Medicare Part D prescription drug plan. Depending on your income, the co-payments for drugs may vary from $0 to $11.20 per drug: most generic drugs have $0 copayment.

Does Community y Más have deductibles or copays for medical services? 

There are some Medicare covered benefit copayments for services such as Emergency Services, Worldwide Urgent Coverage, Medicare-covered Ground Ambulance Services among others.

What are the added benefits of Community y Más? 

In addition to your current Medi-Cal and Medicare benefits, Community y Más includes:

• Worldwide emergency services $50,000 maximum (excludes transportation back to the U.S.).

• Up to $300 for eyeglasses (frames and lenses) or up to $300 for contact lenses every year. 

You will also have access to a Member Services Representative 24 hours a day, seven days a week!
